Add "find or create" functionality

This is a small patch, so I didn't feel the need to fork and pull and all of that.
Attached is a patch for find_or_create and a basic spec for it (that is passing). All unit specs pass on my system with this.

+1 for this, but one hesitation - is find_or_create the best terminology for this? (it sounds a little AR-like).
What about first_or_create?
